Test Ordering and Shipping Instructions

Transplant Genomics distributes TruGraf® Kidney, Viracor TRAC® Kidney, and research sample collection kits to transplant centers and specialists’ practices, or, upon a physician’s order, mails the sample collection kit directly to the patient’s home for use in a commercial blood draw laboratory. Kits may also be drawn by our qualified phlebotomists at the patient’s home through the Labs@HOME mobile phlebotomy service.

Upon presenting the sample collection kit and Test Request Form (TRF), the phlebotomist will:

1. complete the Test Request Form
2. draw the patient’s blood according to the relevant phlebotomy instructions
3. mail the completed Test Request Form and sample collection kit to our CLIA lab for processing.

The sample collection kit contains the proper tube and shipping supplies to stabilize the sample and prevent RNA degradation prior to analysis.

The sample collection kit and complete Test Request Form are received by our CLIA-certified laboratory. Results are returned to the designated clinician within forty eight hours following receipt of sample.

There are multiple ways to order sample collection kits: for a single test, a multiple kit bulk order for laboratory automation, and up to 24 month standing orders.

• Paper Requisition: Transplant Genomics’ tests can be ordered manually by requesting an editable Test Request Form via download here, or via email at info@transplantgenomics.com.
• By Phone: Contact us at 1-844-878-4723.
• Online Order: Transplant Genomics’ tests can be ordered and results accessed through our online portal, Retriever. Access to Retriever can be arranged through your Regional Transplant Manager.
• Interface: Transplant Genomics’ tests can be interfaced through a bi-directional HL7 interface with your EMR system. Development of a custom HL7 interface can be arranged through your Regional Transplant Manager.
